The purpose of this business plan is to raise $500,000.00 for the development of a diagnostic and Interventional Radiology center, AZ MED, LLC. (“AZ MED” or “the Company”). The Company an Arizona LLC that will provide diagnostic radiology services including MRI, CT scan, Ultrasound, Fluoroscopy, and interventional radiology. The Company was founded in 2015 by John Anderson, Victoria Baron, and Mike Davis.
AZ MED operates out of a modern and spacious 12,500 square foot facility in Gilbert, Arizona, only 22 miles southeast of Phoenix, Arizona. Gilbert is one of the fastest growing cities in the U.S. with a population of 240,000 and an average household income of $82,000. The Company’s facility has recently received over $1,500,000 worth of tenant improvements. The facility will house state of the art medical imaging equipment and as many as 24 patient treatment rooms.
AZ MED has secured the necessary patient referral sources and volume to quickly achieve profitability as outlined in the financial projections contained herein. This patient volume is anchored by the diagnostic imaging business as well as the recruitment of interventional radiologists by the Company’s founders to practice at the AZ MED facility. An additional upside is the fact that this facility is also an ideal location for multi-specialty medical practices, including orthopedics, internal medicine, podiatry, pain management, neurology, physical therapy, and chiropractic services.
The Company is seeking $500,000 to fully fund the operation’s startup costs and its initial period of operation. The Company is offering up to 5,000 Class B LLC membership units. Each unit is priced at $100.00. Minimum investment is one membership share defined as 1,000 units or $100,000. These units are non voting and include a 8% coupon.
