Description: Summary: Personalized mobile phone-, web-, and workplace kiosk-based chronic disease management for employers, health plans, and physician groups. Business Summary: Healthcare is traditionally focused on episodes of acute illness and disease. This focus promotes uncoordinated care and increased costs. EosHealth has created a disease management and informatics platform that is scalable yet personalized and uses patent-pending wireless devices that communicate with mobile phones, the web, and worksite kiosks that create a personalized health experience and that coordinate care among health plans and providers. Our product not only leads to improved outcomes but also significant ROIs for employers. Management: The team has a proven track record in healthcare. Kim founded DiabetesAmerica, a network of diabetes centers. Bob serves a Medical Director to several Fortune 50 Companies. Alex is the creator of electronic medical record and information system for Trilliant and GE Healthcare. Our Board members are distinguished individuals in tele-corporate, and disease management. Customer Problem: Chronic disease consumes up to 50% of an employer’s healthcare costs. To reduce these costs, employers have used telephonic disease management programs. Such inflexible, one-size-fits-all programs have led to low employee engagement and marginal reductions in costs or improvement in outcomes. Products/Services: EosHealth sells a unique product of personalized disease management services, focused on diabetes, driven by our patent pending wireless biometric devices and real-time outreach and support that proactively engage employees. Our disease management platform, Progressive User Personalization, focuses on 1) unique personal attributes, not group characteristics to drive the individual’s program, 2) anywhere-anytime access via their mobile phone, web, or worksite kiosk, 3) coordination of care among the users, their doctors, and their health plan, and 4) meaningful interventions that create points of engagement with users that are driven from our analytical database. These all serve to reduce short and long term healthcare costs. Target Market: Our market is large employers (15K+ FTEs) and health plans (100K+) that need to control healthcare costs and absenteeism due to chronic disease (diabetes, hypertension) and need specialized prevention programs (weight management, pre-diabetes). The total addressable market is $15.3B. The served available market is $3.75B and growing (e.g. diabetes at 12%/year). Customers. We have several successful contracts including Harris County, HCHospital District, HISD, and Office Depot. We have many new 15K+ FTE employers beginning Q1 and Q2 and a robust pipeline for 2011. Sales/Marketing Strategy. EosHealth has a scalable model supported by multiple customer acquisition channels. Customers are acquired via large insurance companies and benefit consultants who promote our integrated health management product and via large corporations directly. We are a designated chronic disease management solution for several major health plans. We also acquire customers through durable medical equipment and pharmacy vendors. There are strong incentives to join our program including free supplies, reduced premiums, cash incentives, or selected product discounts. Business model: EosHealth is paid under health insurance contracts and includes service and supplies unique to our wireless devices. Our bundled service/supplies/incentives offering gives employers an immediate cost saving. Competitive advantage: 1) Technology: carrier and wireless devices integrated into our real time Care EngineTM, informatics, on the fly clinical mobile flows, 2) health plan credentialing and integration, 3) IP, 4) customer engagement and tracking, and 5) scalability.
